New Referral Hub helps coordinate referrals between patients, families, and healthcare providers. We are not a direct medical provider, insurance plan, or emergency service.
Submitting a referral through this site does not guarantee:
- acceptance by any provider or agency,
- coverage by insurance, Medicare, or Medicaid, or
- emergency or same-day care.
Providers in our network make their own clinical and admission decisions based on their criteria, capacity, and the information provided.
Information you submit
The referral form collects information needed to coordinate care — patient details, contact information, insurance, the service requested, and any documents you choose to upload (such as a face sheet, medication list, discharge summary, labs, or orders).
This information is used to review the referral and connect the patient with appropriate providers in our network. We share only the information necessary for that purpose.
Protected health information
We treat patient information submitted through this site as confidential and transmit it over secure (HTTPS) connections. New Referral Hub follows HIPAA standards for handling protected health information.
By submitting a referral, you confirm that you have the authority to share the patient's information for the purpose of care coordination.
